                 CHU FÉLIX GUYON







                 Upon admission


                 All necessary information is collected on your arrival :
                     A medical certificate (or a letter from your doctor) requesting your admission.
                    Identification card (or passport) and further proof of your home address.
                    Your insurance cards (NHI registration card/number for UK residents).


                 While you are at CHU Félix Guyon


                     The staff will give you all the information you need, you only have to ask.
                    You may request the help of an interpreter for ease of communication.
                     A social worker is available to assist you.


                 Rules

                     Patients with no insurance are charged at a rate
                   of 1 500 euros per day. Uninsured patients, or
                   those for whom  insurers confirmation of cover
                   has not been received, are required to pay a de-
                   posit.
                     Staff members will make a telephone available
                   in your room on request.
                     Patients may have up to 3 visitors (except in ex-
                   ceptional cases) over the age of 15 at any one
                   time, although morning visitors are not permit-
                   ted. Children may visit, but only with special per-
                   mission.


                 Health information

                     Your doctor will impart to you information
                   concerning your health, and any further care required. Information concerning child patients will be
                   given to their parents.
                     For reasons of confidentiality, staff are  not allowed to give information to third parties without the
                   express permission of the patient.


                 Going home

                     The doctor will decide when you are ready to leave the hospital. Parents of children will be informed
                   when their children are ready to leave, so that they can make appropriate arrangements.
                     CHU Félix Guyon accepts no responsibility whatsoever for patients who discharge themselves against
                   the advice of their doctor. Patients taking this action will be obliged to sign an undertaking absolving
                   CHU Félix Guyon of liability.
                     Either the patient or a close family member must check out at reception before leaving CHU Félix
                   Guyon, so that we may be sure that your records have been fully completed.
